About US
Agape Awesome Arrivals is Prenatal Care and Child Care Coordination Agency that provides services to pregnant women and mothers in Milwaukee County who have recently given birth. Our company makes efforts to establish working relationships with local health care and social services providers early identification of pregnancy programs in addition to local organizations dedicated to the wellness of women and families. Agape Awesome Arrivals aims to connect women and families with resources to meet educational, health and wellbeing according to the needs expressed.

PNCC
Prenatal Care Services
A Brief Overview of Prenatal Care Coordination (PNCC) Services:
PNCC services help a member and, when appropriate, the member's family, gain access to medical, social, educational, and other services related to the member's pregnancy. Wisconsin Medicaid PNCC services are available to Medicaid-eligible pregnant women with a high risk for adverse pregnancy outcomes during pregnancy through the first 60 days following delivery.
PNCC services include all of the following:

Ongoing care coordination and monitoring

Health education and nutrition counseling services (for members with an identified need)
Prenatal Care Coordination Goal

The goal of the PNCC benefit is to improve birth outcomes among women who are deemed at high risk for poor birth outcomes. The main objectives for obtaining this goal include ensuring that women at high risk:

Are identified as early as possible in their pregnancy

Receive individual psychosocial support and services

Receive early and continuous prenatal care services
-
Receive necessary health and nutrition education
-
Are referred to available community services, as appropriate
-
Receive assistance in accessing and obtaining needed health and social services
CCC
Child Care Coordination
A Brief Overview of Child Care Coordination (CCC) Services:
CCC services help a member and, when appropriate, the member's family gain access to and coordinate a full array of services, including necessary medical, social, educational, vocational, and other services. ForwardHealth CCC services are available to BadgerCare Plus and Medicaid members in Milwaukee County and the city of Racine who receive a Wisconsin DHS-approved initial risk assessment within eight weeks following the birth of a child. Members in Milwaukee County qualify for CCC services until the child's seventh birthday. Members in the city of Racine qualify for CCC services until the child's second birthday.
CCC services include all of the following:

Child Care Coordination Goal
​
The CCC benefit extends the Medicaid PNCC benefit in Milwaukee County and the city of Racine. The goals of the CCC benefit are to promote positive parenting, improve child health outcomes, and prevent child abuse and neglect.
The main objectives for obtaining these goals include the following:

Improving family functioning

Improving parenting skills and positive parenting outcomes

Increasing members' understanding of infant and child development

Increasing members' access to and appropriate use of the health care delivery system

Improving employment outcomes

Encouraging planned pregnancies

Improving future birth outcomes
