Pregnancy is often filled with questions. Whether you have a specific concern or are excited about the options available, we are here to answer your inquiries. We have compiled a list of FAQs that we think will help.

What Could My Prenatal Care Coordination Services Include?

Prenatal care coordination services will be a little different for each person. Prenatal care coordination services might include:

  • Completing a pregnancy questionnaire with your care coordinator

  • Creating a care plan that has supports and services based on your needs

  • Meeting regularly to go over your care plan and make changes 

  • Scheduling health care appointments  

  • Connecting you to community-based services such as FoodShare, housing, and transportation

  • Going to classes on health education and nutrition - individual or small group

What Will My Care Coordinator Do?

  • Give you one-on-one support to help you with the challenges of pregnancy

  • Meet with you regularly at a time and place that you choose

  • Talk with you about your prenatal care coordination services and answer questions you have

What Questions Can I Ask My Care Coordinator? 

  • What health visits are needed and covered by Medicaid?

  • How often should I go to the doctor?

  • How can I get to the doctor?

  • What are signs of labor?

  • What will my baby need after they are born?

  • And More!

When Should I Share My Medicaid ID?

  • Do not give out your Medicaid ID number (the 10-digit number on your ForwardHealth card) to any provider until you start receiving services. You should report any concerns about how your personal information is being used. You should also report if you gave out your Medicaid ID and never received services. Call 1-877-865-3432 or file a report with the Office of the Inspector General.
